P11D Information Request

 

Please complete all the questions below and submit the form:

P11D Information

  • Do you provide any assets to the employees/directors either free of charge or under market value (such as cars, furniture, kitchen appliances etc)?
  • Are payments made on behalf of employees/directors (such as utility bills paid)?
  • Do you provide vouchers or credit cards to the employees/directors? (Please provide the value of the vouchers or private credit charges paid).
  • Do you provide living accommodation to any employees/directors? (Please contact me for more information).
  • Do you provide mileage allowance (where payments exceed the HM Revenue and Customs (HMRC) Authorised Mileage Rates)?
  • Car and fuel. If there has been a change of car, please provide a copy of the new car purchase invoice if available. If not, we require exact details of make, model, registration and the cost of any non-standard accessories. Please be aware that even if the vehicle running costs are debited to a director’s loan account there is still a reportable benefit in kind.
  • Do you have any vans, including twin-cab pickups? (Please contact me for more information).
  • Do you provide any interest free loans to your employees/directors? (Please contact me for more information).
  • Do you provide any private medical treatment or insurance to your employees/directors?
  • Have you provided relocation expenses to your employees/directors? (Please identify individual costs).
  • Do you supply any services to employees/directors (for example a driver or gardener)?
  • Are there any assets placed at your employees/directors’ disposal (such as holiday home or boat)?
  • Do you provide any share schemes to your employees/directors? (Please contact me for more information).
  • Do you provide any subscriptions or professional fees to your employees/directors?
  • Do you provide any other benefits to your employees/directors? (Please contact me if you are not sure if you need to include an item).
  • Do you provide travel and subsistence payments to your employees/directors? (Please provide details only if these exceed HMRC’s benchmark rates and you have not agreed a bespoke rate with HMRC).
  • Do you provide entertainment expenses exceeding £150 per person to your employees/directors? (If these are not wholly for business purposes.)
  • Do you make payments for use of home telephone to any employees/directors? (Please let us know the split between rental and calls).
